Determination of “Evaluation Only”

The therapist, after completing an evaluation, may determine further intervention is not necessary beyond evaluation. This may occur, for example, if the patient was referred for an evaluation due to a suspected decline and a thorough assessment revealed no functional change or the patient may benefit from skilled services and the patient is unwilling to participate in an active treatment program. When an evaluation only is determined, it would NOT be appropriate to provide subsequent treatment since a plan of care and goals were not established. Only evaluation code/time would be billed.
